This tutorial will guide you through configuring custom options for questions on forms, including how to manage element visibility and titles effectively.

To begin, navigate to the Settings tab.
Then proceed to the Forms and Consent section.

Select a sample form to customize; for this tutorial, we will use the Intake Form.

Next, identify the question you wish to customize; for example, we will customize the Allergies question.
Click on the question to edit it.

In the right sidebar, you will find options for Element Visibility, Element Titles, Element Requirement, and Element Choices.

To specify Medicine Allergies, ensure that the other relevant allergies are not displayed.
Simply click the necessary options to achieve this configuration.

To verify the changes, click Preview.
You will observe that the form now displays Medicine Allergies without showing other relevant allergies.

You can also modify the question titles.
For instance, change the title to Important Medicine Allergies.

Additionally, you have the option to change the element choices.
For example, check if the patient is also allergic to any specific substances.
You will see that the title has been updated accordingly.

Finally, observe that Drug B is now included as an option in the form.
