How to Create a New Intake Form

Making a form from scratch using custom and pre-set questions

Creating a New Intake Form

  1. Accessing Forms Tab

    • Go to the Forms tab on the Forms and Consents settings.

  2. Initiating a New Form

    • Click on New Form to start creating a new intake form.

  3. Building the Form

    • Drag and drop elements from the left into the center to begin constructing the form.

    • Customize with preset options or create custom questions by dragging the desired elements.

  4. Organizing the Form

    • Arrange form elements by dragging them next to, under, or over each other as needed.

  5. Setting Question Requirements

    • Make questions required by clicking the required button.

    • Preset questions on the bottom left will automatically track back to the medical info tab on the patient profile.

  6. Naming and Saving the Form

    • Give the form a name by editing the text field at the top.

    • Save the form by clicking on the save icon when satisfied with the form.

  7. Associating Services with the Form

    • Scroll down on the right in the settings section to select services associated with the form.

    • Choose services by clicking from the list or typing to search. Multiple services can be selected.

    • Optionally, make the form required for all services using the checkbox.

  8. Setting Form Frequency

    • Determine how often the patient should fill out the form.

Be sure to save the form after any edits have been made, including associating it with services or changing the required frequency.
