1. Open Chart export
Open the patient’s profile.
Select Action → Send Patient Records.
Choose Chart export.
2. Choose what to include
Start with Full record, Face sheet only, Referral packet, or Custom. Review the selections before continuing.
Turn off any sections you don’t need.
Use the arrows to change the order of the sections in the PDF.
Keep All dates or select Set date range for sections that support date filtering.
Check the record counts shown for each section.
Appointments and receipts can also be selected individually. Choose Select for the section, then search, page through the results, or select all matching records.
What’s included? Available sections include the face sheet, appointments, receipts and quotes, clinical notes, documents, and signed consents. Documents include uploaded files, received faxes, and good-faith exams.
Important: Appointment notes stay internal, and office notes are never exported. The face sheet shows the patient’s current details and is not filtered by date. Date ranges include both the start and end dates; check the time zone shown in the export.
3. Add face sheet notes and referral details
On Face sheet notes, add any orders or instructions for the receiving provider. These appear in an Orders & notes box on the first page of the face sheet, along with your name and the date.
If needed, complete the referral details so the recipient knows who to contact. You can select a practice location to fill in the address, phone, and fax, then edit those details.
4. Choose how to deliver the export
On Delivery, choose one of these options:
Email me the PDF: Receive an email link to the PDF. The link expires in 24 hours.
Save to chart documents: Add a permanent copy to the patient’s Documents.
Fax: Send the export by fax. A copy is also kept in the chart.
To make a copy available to the patient, select Also share the PDF with the patient in the portal. This adds a chart document copy that the patient can see. Leave this option off if you do not intend to share the export with the patient.
5. Review and submit
Review the included sections, selected records, date ranges, face sheet notes, and delivery method.
Go back and make any changes needed.
Use the final save or send action to submit the export.
The PDF is generated in the background. You’ll receive an email when it’s ready, and the export is logged in the patient’s activity.
