How to Require Provider or Witness Signatures on Consent Forms

Learn how to add a field for a provider and/or witness signature in Prospyr digital consents

1. A. In the applicable consent form, click the signature panel button

3. Change signature type from patient to provider or witness

ℹ️ It is not recommended to make provider or witness signatures required, as it will prevent the patient from initially submitting the form if any required signatures have not been added.

4. A. Click on Settings

B. Verify that Requires Provider Signature and/or Requires Witness Signature are toggled

Once confirmed, you can click save changes to save the consent.
