How to create, edit, and use consents

1. To see outstanding consents, click on the applicable appointment

3. Scroll down to the consent section. Any item appearing in red is an outstanding consent that must be completed. Click on the name of the consent to be taken to the form.

4. The patient will be prompted to print their name, sign, and date. When done, click complete to be taken back to the previous page.

Note: In order for a consent to be considered complete, patients must sign AND date. If your consent form is only configured with the signature prompt, you will notice that the consent will remain in an incomplete status regardless of if the patient has signed or not. The name/signature/date prompt will need to be added in the consent configuration.

5. Another option to access all outstanding consents is to click on the Complete All Outstanding button. This will carousel through any outstanding consents and forms, ensuring nothing gets missed.

6. To manage consents, click Settings on the left

8. You will see a list of current consents. To edit, click the title of the applicable consent. To create a new consent, click New Consent

9. To start building your consent form, select Add Question

10. Click on the dropdown to change the type of input

11. Select which option you'd like

Initials- Prompts patients to enter their initials

Name / Signature / Date - Prompts patients to enter their printed name, sign, and date

Media Consent - Populates a standard media consent

Consent Text- Allows you to copy and paste your consent document, or type a new one

Signature- Prompts patients for signature only

12. To add your own text with the Consent Text Feature, click on the Show Panel button on the right

13. The HTML Markup box is where you will either copy and paste, or type your desired text

Ensure that the visible box is checked, otherwise the text will not appear on the patient's view of the form.

14. Click on Hide Panel once your text has been entered

15. To continue the form and add another section below the existing one, click Add Question

This additional section can be used to prompt for a patient signature

16. If you'd like the step to be required, make sure the required button is toggled.

It will show orange when enabled.

17. When you're happy with the content and want to save, click configure consent

18. Here you can give the consent a title, and add a description

19. You can also toggle which options apply to your new consent form, and select which (if any) services the consent is applicable to

Partial Media Consent- Consent for patient photos to be used in medical contexts but NOT social media/advertising

Full Media Consent- Consent for patient photos to be used in medical contexts AND social media/advertising

Requires Provider Signature- The provider will be prompted to sign

Requires Witness Signature- Someone who is not the patient will be required to witness the patient sign, and sign their own signature

Required Services- Selecting services here will ensure that the consent form is only tied to those services

Recurrence- Allows you to select how frequently you'd like patients to sign

You can also toggle which options apply to your new consent form, and select which (if any) services the consent is applicable to

20. Click on Save consent when done
