How to Add an Additional Form/Consent to an Appointment

Adding an extra form or consent that isn't already associated with an appointment

Adding Additional Consent or Form in Loom

  1. Locate Document Section

    • Scroll down on the appointment to find the document section.

  2. Add New Consent

    • Click on the pencil icon in the consent box to add a new consent.

    • Click on "Add New Consent"

    • Search for the consent to add, then click save changes.

  3. Add New Form

    • Click on the pencil icon in the form box to add a new form.

    • Click on "Add New Form"

    • Search for the form to add, then click save changes.
